Appointments 

 The patient needs to call to make their appointment and we always try to see them en the same day if possible, when you call make sure to provide with your personal information, address, phone and insurance. In order to get the most effective information to treat you, we recommend you bring the following:

-         Photo ID

-         Insurance Card

-         Required co-pay or deductible

-         Most recent lab reports, xrays, PAP smear results and mammograms

-         All current medication bottles or names and dosages of all medications you are taken specially if you need
     a refill.
